4 Requirements to Qualify for VA Improved Pension with Aid and Attendance Benefits

In this post, I’m breaking down the four main requirements you must meet to qualify for VA Improved Pension with Aid and Attendance benefits. These benefits can help veterans and their surviving spouses receive financial support for essential care needs.

1. Military Service Requirement

The first requirement is related to military service. The veteran must have served at least one day on active duty (not Reserve duty) during a recognized wartime period. This does not mean the veteran had to serve in active combat—clerical or other non-combat roles still count as long as they were active duty. The veteran must have served at least 90 consecutive days, including that one day during wartime, and must not have received a dishonorable discharge.

2. Medical Need Requirement

The second requirement focuses on medical need. The benefit levels increase depending on the level of care required:

  • Basic level: Over age 65.
  • Housebound level: Unable to leave the home without assistance, needing help with at least one activity of daily living.
  • Aid and Attendance level: Needing the assistance of another person for two or more activities of daily living (such as bathing, dressing, or eating).

This benefit is designed for veterans who require immediate care or for their surviving spouses who meet these criteria and whose spouse met the military requirement.

3. Income Requirement

The third requirement involves income. To receive the full benefit, your gross income (such as Social Security, pensions, or investments) must be offset by your unreimbursed medical expenses. These expenses may include:

  • Private caregivers or in-home care services
  • Assisted living expenses
  • Payments to non-spouse family caregivers (such as children, nieces, or nephews)
  • Ongoing health insurance premiums

If these unreimbursed medical expenses exceed your gross income, you may qualify for a higher level of benefit.

4. Asset Requirement

The fourth requirement is the asset test. According to VA guidelines, they generally are not concerned if total assets are below $80,000. Your primary residence, one vehicle, and personal belongings are not counted. While there is no official “look-back” period like Medicaid has, it is still wise to maintain assets on the lower side for faster approvals and fewer complications.

Final Thoughts

Meeting these four requirements—military service, medical need, income limits, and asset limits—can open the door to valuable Aid and Attendance benefits. These benefits are designed to help veterans and their surviving spouses access the care they need without unnecessary financial strain.
